Ampicillin
Blocks transpeptidase cross linking in cell wall.
Drug of choice for: Listeria, Enterococcus.
Also kills: H. flu, E. coli, proteus, salmonella.
Amp + gent good empiric coverage for GNRs.
Can cause hypersensitivity rx, rash, pseudomembranous collitis.
Antipseudomonal penicillins
Ticarcillin, carbenicillin, piperacillin.
Work like penicillin.
Use with a clavulanic acid to take care of psuedo (and other GNRs).
Can cause hypersensitivity rx.
1st generation cephalosporins
Bactericidal beta-lactams that inhibit cell wall synthesis.
Cefazolin and ceph____'s.
Good for GPCs and PEcK (proteus, E. Coli, Klebsiella), sometimes used before surgery
Some X-rx with penicillin, can have nephrotoxicity, and those with methylthiotetrazole groups can have disulfaram effect
2nd generation cephalosporins
Bactericidal beta-lactams that inhibit cell wall synthesis.
Ce__ fox/fam's 2nd fa/te was fur.
1st gen coverage (GPS + PEcK), add H. flu, enterobacter, neisseria, and serrata (HEN PEcKS)
Some X-rx with penicillin, can have nephrotoxicity, and those with methylthiotetrazole groups can have disulfaram effect
3rd generation cephalosporins
Bactericidal beta-lactams that inhibit cell wall synthesis. This gen gets through BBB.
Cefixime, all others have t, like ceftriaxone
Pretty big GN gun. Ceftriaxone is first line for both neisserias. Ceftazadime will work on pseudomonas.

4th generation cephalosporins
Bactericidal beta-lactams that inhibit cell wall synthesis.
Cefepime.
Big gun, inc psuedomonas (GN and GP)
Some X-rx with penicillin, can have nephrotoxicity, and those with methylthiotetrazole groups can have disulfaram effect
Aztreonam
Cell wall inhibitor (binds to PBP3) but NO penecillin x-rx. Syngergystic with aminoglycosides, or good for people who can't take them (e.g. renal insufficieny). Useless against GPs or anarobes.
Good for klebsiella, pseudomonas, and serattia.
Well tolerated.
