Fentanyl - Class, Trade Names

Fentanyl - Description
Potent, Centrally acting opiod analgesic.
Fentanyl - Action/Mechanism
Potent (81 more times potent than morphine) mu opiod receptor agonist. Fentanyl has greater reduced emetic effect than other opiod analgesics (morphine meperidene) and no significant histamine effect (no induced histamine release).
Fentanyl - Uses/Indications
Control of pain associated with cardiac and musculoskeletal pain.
Fentanyl - Contraindications/Precautions
Hypersensitivity to fentanyl or other fentanyl analogues. May cause muscle rigidity (including muscles of respiration) if given in high dose or too rapidly.
Fentanyl - Adverse/Side effects
CNS: CNS depression
CV: hypotension, bradycardia
GI/GU: vomiting, nausea, diarrhea
Resp: Respiratory depression, apnea, depressed C02 sensitivity.
Fentanyl - Pharmacokinetics
Onset: <3 min IV
Peak effect: 5 to 15 min IV
Duration: 2hr
Half Life: terminal elimination half life 3.5hr
Fentanyl - Interactions
Possible additive or synergistic effect with other opiod analgesics. Has been shown to produce hypertension when used with droperiol. Use with caution , or reduce dose when used with tranquilizers, barbiturates or other CNS depressants.
Fentanyl - Dosage
Adult dose: 1 mcgm/kg initially then 0.5 mcgm/kg every five minutes to a max of 3.0 mcgm/kg
or
100 mcgm slow IV push to 10-20 mcgm/minute.
Pediatric dose: 1 mcgm/kg to a max of 3.0 mcgm/kg
Fentanyl - Administration
IV preferred, slow push. May be given IM if IV route is not available. Use 1/2 dose in patients over 65 years of age. Continuous monitoring is essential
Fentanyl - Notes
Protect from light and heat. Pregnancy category C - has been shown to be detrimental to fertility and cause fetal harm.

Morphine - Description
Opioid analgesic. Central nervous system depressant with significant hemodynamic properties.
Morphine - Therapeutic Action/Mechanism
Acts by stimulating opiate (endorphin) receptors in the central nervous system producing analgesia and sedation.
Directly acts on vascular smooth muscle.
Increases peripheral venous capacitance, decreases venous return.
Morphine - Uses/Indications
Analgesia: moderate to severe pain of cardiac, musculoskeletal or other nature.
Pullmonary edema associated with congestive failure.
