Pancreas
*both endocrine and exocrine gland
Endocrine
*secretes hormones directly into the blood or lymph
Exocrine
*secretes digestive enzymes into the pancreatic duct
Where is the pancreas situated?
*situated obliquely in the anterior portion of retroperitoneum
*non-encapsulated
Blood supply to pancreas
*branches of GDA
*splenic artery
*SMA
-all of which should be identified
Parts of the pancreas
*divided into :
- uncinate process
- head
- neck
- body
- tail
**BOUNDARIES ARE RELATED TO THE SURROUNDING ANATOMY**
Measurements of Pancreas
Head-->2.5 cm (includes uncinate process)
Body-->2.0 cm
Tail-->2.0 cm
*taken in AP diameter
Uncinate process
*beadlike projection extending posterior and medial from the head
**seen just POSTERIOR to the PORTAL SPLENIC CONFLUENCE**
Head
*situated to the right and inferior
*largest AP diameter
*located anterior to IVC and RRV; medial to 2nd portion of duodenum
Within the head:
*GDA (anterolateral)
*CBD (posterolateral)
**both appear as sonolucent structures within the head on transverse scans
Neck
*located just anterior to convergence of splenic vein and SMV (portosplenic confluence)
Body
*bounded anteriorly and superiorly by portions of the :
- stomach(lies in lesser sac)
-duodenum
-left lobe liver
Tail
*extends from body to LUQ
*superior to head
*most difficult to image*anteromedial to LK*medial to splenic hilum*inferior to splenic artery*anterior to splenic vein
Pancreatic Ducts
*within the pancreatic parenchyma: - lobules of acinar cells that secrete digestive enzymes into surrounding ducts - these small ducts that surround each lobule merge to form the main pancreatic duct
Wirsung's Duct
*AKA = Main Pancreatic Duct*small exocrine ducts merge into main pancreatic duct*runs entire length of pancreas**SHOULD NOT EXCEED 2mm****Joins distal CBD to enter 2nd portion of duodenum through Ampulla of Vater**
