Why does the WHO recommend exclusive breast feeding for 6 months
Complete food, bioavailable long chain PUFs, arachidonic acid and DHA for brain and eyes. protection against bacterial and viral infection, less GI ilness, reduces risk of cot death, enhances developmental and emotional factors, long term health - reduces likelihood of asthma, obesity, diabetes, increased BP
Benefits to mother by breast feeding
Reduced incidence of disease; depression, diabets, ovarian and breast cancer. assists post natal wt loss, financial, convenience, breast feeding support grps-eg La Leche
Problems with infant formulas
Heat treated cow's milk which ruins immune properties., cows milk higher in sodium than breast milk.
Benefits of infant formulas
Know how much getting, anyone can feed baby, expressingbreast milk in public, self conscious, only suitable alternative to breast milk during 1st year of life
Infant follow on formulas
Higher iron content than infant formula milk. may prevent IDA in certain infants eg Asian and those living in inner cities if used cows milk alternative after 12 months age. Marketing ploy, should be promoting cows milk or ordinary balanced diet + extra vit D
Cows milk
Should not be givenas main drink before 12 months of age
Gluten and breast feeding
Avoid both early ; before 4 months, and late; 7 months or over introduction of gluten . introduce it while baby still breast fed
Problems with weaning too early
Neuromuscular coordination not sufficiently developed to enable transfer of a food bolus from front to back of mouth or safe swallowing , high solute load for immature kidneys, immature GI tract eg production of digestive enzymes not fully developed and increased permeability of immature GI tract, increases risk of allergic rxn to food antigens
Marketing infant formulas
Should not be promoted in healthcare facilities, not advertised to public, no free samples to mothers, no company nurses should advise mothers, no gifts to healthcare workers, no idealing words or pictures on products, info to healthcare workers should be scientific and factual, explain benefits of breast feeding and costs and hazards of artificial feeding on packaging. unsuitable products like condensed milk not promoted for babies
Infant formula in comparison to cows
Decreased protein, increased carb, decreased sodium, decreased iron.
Method to reconstitute infant formula
1 level scoop to every 30ml (1fl oz) cooled boiled water. warm milk in bottle by standing in hot water, shake a few drops onto wrist.
Reconstiting infant formulas, strict hygeinic practice
Bottles, teats, bottle caps, rinsed after use, washed in hot water with detergent, rinsed sterilised. cooled boiled water to reduce bacteria. bottled water must be boiled as not sterile. sodium content should not be more than 200mg/l some spring waters exceed this level. Water temp 70 degrees ensures final pasteurisation.
Why do we need to wean and when ideally
For energy, nutrient needs too large for milk alone, iron store depleted. developmental; biting, chewing. wean 4-6 monthsas late weaning can delay development in eating behaviour and speech
Problems if wean too late
Nutrient deficiency, underdeveloped feeding skills, fussy/faddy eaters, may increase allergies
Weaning points of advice
Mothers need nutritional advice and supplements. if weaned at 6 months need to love quickly to mixed diet, parents who wish to wean earlier than this should be encourged to view 4 months as earlist
