Whats the difference between TEWL between term and preterm baby
TEWL is 15 times higher in 25 weeker than a term baby
What is the expected body composition change in preterm and term baby
It is expected for a preterm to lose up to 15% in their body weight and term babies to lose up to 10%.
What happens if excess fluid is given to prem infants to 'prevent' this weight loss
Increases the chance of morbidity, PDA, NEC and chronic lung disease
How is fluid lost in preterm babies?
As babies have larger surface area, more chance of loss, specially in preterms
Each ml of water lost through the skin is accompanied by what?
Its accompanied by 560 calories of heat loss
What needs to be considered when prescribing fluids to neonate?
Fluid in - volume and type of fluid
fluid losses - volume and type (stools, urine, insensible)
disease processes
body weight changes (excess weight gain may be tissue fluid rather than fat or muscle
serum electrolytes
What is the usual 'recipe' for fluids for neonate?
Start with 10% dextrose
may need to change % of dextrose depending on baby (eg small baby may need higher concentration)
How many mls/kg/day should you start with
60ml/kg/day
What are the common clinical problems
Respiratory distress
PDA (patent ductus)
What are some more common clinical problems
Birth asphyxia - anticipate renal failure, restrict fluid (30ml/kg/day), no Na supplementation, expect hypoglycaemia and may need to increase % dextrose
How much does a fetus grow in utero per day?
15g/kg/day, however cannot mimic this after births (aim for more than 20gms/day
What are the energy requirements for term, preterm, IUGR babies?
Term babies - 100kcal/kg/day
Preterm babies - 120kcal/kg/day
IUGR babies - 140kcal/kg/day
