What is Evidence-Based Practice?
|
EBP is a process that supports clinical decision making by allowing the practitioner to make educated and informed decisions using the most available up-to-date information.
|
EBP practice requires: (5-A’s)
|
Asking questions (reflection
Acquiring evidence (database searches
Appraising the evidence (analysis)
Applying the evidence to the particular case.
Assessing if the selected path will give the outcome desired.
|
Creating EBP tools
|
Creating systematic reviews, clinical guidelines, clinical prediction rules and adding to research evidence requires a high level of skill and knowledge of research techniques, statistical analysis, and information synthesis
|
5 minute EBP
|
-Check Clinical Prediction Rules for your
patient. Most CPR matches take less than 5
minutes if you know where to find
-Download helpful information to a PDA or Smartphone for quick use in the clinical setting.
-Email a journal article request to a librarian
|
Less than one hour EBP
|
Read last month’s issue of a professional
journal. Scanning the most recent journal in
the areas of your interest should take less
than an hour, especially if you have already
looked at the Table of Contents from an e
alert.
|
EBP Searches
|
-Searching for valid and reliable sources to answer your evidence-based question is not as simple as finding one or two articles.
- You need to be looking for the different classes or levels of evidence.
-If you find the answer at a higher level of evidence, then you can stop your search and have confidence that you have found the best available answer.
|
What are the four elements that must be integrated to practice EBP?
|
Patients values,
environment,
evidence
circumstance
|
What is the evidence Pyramid
|
The pyramid is ‘worked’ in searches from the top down. If you look under the title of the level you will see the most appropriate database to begin your search. This pyramid is for medicine,
|
Name levels of Pyramid from top to bottem
|
- Systemic Review (chochron based)
- Critically Appraised Topics ( clinincal evidence data base)
- Specialty Specific- POEMS
- Critically Appraised Individual Articles (ACP Journal club)
- Texbooks
- Journal Articles (Pubmed)
|
What is a "Keyword" in EB?
|
- Major concepts or variables of a research problem or topic used to search a database
- May be single terms or phrase
- Most databases have a thesaurus that can be used to identify keywords
- Each keyword used should be listed in a written search plan
|
Recording Search Information
|
- Name of database
- Date of search
- Exact search strategy used
- Number of articles found.
-Percentage of relevant articles found. This information can be stored in a table ORIf you Endnotes be sure your options include all of the following.
