DEA 222
|
The DEA 222 form is used for the purchasing and returning of outdated CII drugs. The DEA 222 form comes in three copies:
1) Blue copy: the purchaser keeps the blue copy on file for at least two years. The pharmacy is usually the purchaser except when the pharmay is sending back out-of-date drugs then it becomes the seller.
2) Green copy: The seller sends the green copy to the local DEA.
3) Brown copy: The seller keeps the brown copy on file.

The DEA 106 form is used to report lost or stolen controlled substances. A DEA 106 form is used when 5% of the yearly product sold is missing.

The DEA 41 form is used to document the destruction of controlled substances.

|
The DEA 224 form is needed for a pharmacy to dispense controlled substances.

|
The DEA 363 form is needed to operate a controlled substance treatment program or compound substances.
