Appling One Knee-High Elastic Stocking

1. Explain Procedure to Client 2. Provide Privacy (curtain, screen or door)3. Lower bed Flat if not already (client needs to be lying down *Supine Position* while appling stockings)4. Turn stockings inside-out5. Place foot of stocking over toes, foot & heel6. Pulls top of stocking over foot, heel & leg7. Move foot & leg gently (avoiding foce)8. Make sure there are no wrinkles or twists & heel of stocking is over heel & toe opening is either over or under toe aea.9. Signaling device is in reach and bed in low position10. Wash Hands

Assist to Ambulate using Transfer Belt
|
1. Explain Procedure2. Before assisting to stand, ensure client is wearing shoes3. Before assisting to stand make sure bed is at safe level & bed wheels are locked4. Assist Client to sitting position with feet flat on the floor5. Apply transfer belt securely over clothing/gown6. Provide instructions to enable client to assist in standing with prearranged signal7. Stand facing client position self with one foot between clients legs8. on count of three, gradually assists client to stand by grasping transfer belt on both sides (clients hands should be on your shoulders)9. Walk slightly behind and to one side of client for a distance of ten (10) feet, while holding on to belt10. After ambulation, assists client to bed and remove transfer belt11. Signaling device is in reach & bed in low position12. Wash Hands

Assist with use of Bedpan
|
1. Explain Procedure2. Provide Privacy 3. Lower Head of the bed 4. Put clean gloves on 5. Place bedpan correctly under client's buttocks6. Remove & disposes of gloves (without contamination self) into waste container & Wash Hands7. After positioning client on bedpan & removing gloves, raise head of bed8. Place toilet tissue & hand wipes with in reach of client, make sure client is instructed to clean hands with wipe when finished9. Signaling device with in reach & client is aksked to signal when finished10. Put on clean gloves and lower bed before removing bedpan (avoid overexposure of client)11. Remove, empties & rinses bedpan and pours rinse into toilet12. Place bedpan in designated dirty supply area13. Remove & disposes of gloves (without contaminating self) & Wash Hands14. Signaling device is within reach & bed is in low position

1. Puts on clean gloves before handling dentures2. Sink is lined and/or sink is partilly filled with water ( before denture is held over sink)3. Rinse denture in tepid tempertature running water 4. Apply toothpaste to toothbrush5. Brush surface of denture6. Rinses surface of denture under tepid temperature running water7. Rinse denture cup & lid8. Place denture in denture cup with tepid temperature water & place lid on cup9. Rinses thoothbrush & places in designated toothbrush basin/container10. Maintain clean technique with placement of toothbrush & denture11. Sink liner is removed & disposed of appropriately &/or sink drained12. Remove & disposes of gloves (without contamination self) & Wash Hands

Donning and Removing PPE (Gown and Gloves)

1. Picks up gown & unfolds2. Facing the back opening of the gown place arms through each sleeve3. Fasten the neck 4. Secure gown at waist making sure that back of clothing is covered by gown (as much as possible)5. Puts on clean gloves,make sure cuffs of gloves overlap cuff of gown6. With one gloved hand, grasps the other glove at the palm, pull glove off7. slip fingers from ungloved hand underneath cuff of remaining glove at wrist, and remove glove turning it inside out as it is removed8. Dispose of gloves (without contaminating self)9. Unfasten gown at neck & waist10. Remove gown without touching outside of gown, hold gown away from body, turn gown inward & keeps it inside out10.Dispose of gown (without contaminatiating self)11. Wash Hands

Dressing Client with Affected (Weak) Right Arm

1. Explain Procedure2. Provide Privacy 3. Ask client which top he/she would like to wear4. While avoiding over exposure of client, remove gown from the unaffected side first, then remove gown from the affected side5. Assists to put the right (affected/weak) arm through the right sleeve of the top before placing garment on left (unaffected) arm moves body gently and naturally, avoiding force6. Finish with clothing in place7. Disposes of gown into soiled linen container8. Signaling device is within reach and bed in low position9. Wash Hands

Feeds Client Who Cannot Feed Self

1. Explain Procedure2. Pick up name card from tray & ask client to state name3. Adjust bed to upright position (45-90 degrees)4. Place tray where it can be easily seen by client5. Clean client's hands with hand wipe before feeding6. Sit facing client, tell client what foods are on tray & ask what client would like to eat first7. Using Spoon , offer client one bite of each type of food on tray, telling client the contents of each spoonful8. Offer beverage at least onece during meal9. Make sure client's mouth is empty before offering next bite of food/beverage10. End of meal wipe client's mouth & hands with hand wipe11. Remove food tray & place tray in dirty supply area12. Put on clean gloves6.Begin with eye's, wash eye's (No Soap), using different area of washcloth for each eye, wash enter and outer aspects then wash face7. Dry face with towel8. Expose one arm and place towel underneah arm9. Apply soap to wet washcloth10. Wash arm, hand & underarm, keep rest of body covred10. Rinse and dry arm, hand & underarm (move body gently & naturally avoiding force)11. Put clean gown on client12. Empty, rinses, & dry basin & place in desinated dirty area13. Dispose of used gown & linen (avoid contact with your clothing)14. Remove & dispose of gloves (without contaminating self)15. Wash Hands16. Signaling device in reach & low bed

1. Explain Procedure2. Provide Privacy3. Lower head of bed 4. Client is covered & loosen top linen from end of bed5. Raise side rail on side client will move to & ask client to roll to side rail6. Loosen used bottom linen & work towards center of bed7. Place and tuck clean bottom linen & tucks under client8. Have client roll over to clean linen side9. Raise side rail then move to other side of bed10. Removes used bottom linen11.Pulls & tucks in clean bottom linen, finishing with bottom sheet (wrinkle free)12.Client is covered with clean top sheet & bath blanket/used top sheet has been removed13. Change pillow case14. Linen is centered and tucked at foot of bed15. Avoid contact between clothing & used linen, dispose of used linen into soiled container (DO NOT PUT ON FLOOR)16.Signaling device is within reach & bed lowered 17. Wash Hands

Measures and Records Blood Pressure

1. Explain Procedure2. Wipe bell/diaphragm & earpieces of stethoscope with alcohol3. Client's arm is positioned with palm up & upper arm exposed4.Feel for brachial artery on inner aspect of arm, at bend of elbow5. places blood pressure cuff snugly on client's upper arm, with sensor/arrow over brachial artery site6. Stethoscope is in place7. Inflate cuff between 160mm Hg to 180mm Hg (If beat heard immediately upon cuff deflation, completely deflate cuff) Re-inflate cuff to no more than 200mm Hg or brachial pulse was last heard or felt8. Deflate cuff slowly and notes the first sound (systolic reading), and last sound (diastolic reading) If rounding needed, measurements are rounded UP to the nearest 2mm of mercury9. Remove cuff10. Signaling device within reach11. Wash Hands12. Record both systolic and diastolic pressures each within plus or minus 8mm of evaluator's reading* This is a on-step blood pressure procedure.

1. Put on clean gloves 2. Pour contents of bedpan into measuring container (without spilling or splashing urine)3. Measure the amount of urine at eye level with container on flat surface4. After measuring urine, emptie content of container into toilet5. Rinse measuring container & pour rinse into toilet6. Rinse bedpan & pour rinse into toilet7. After rinsing equipment remove & dispose of gloves8. Wash Hands9. Record contents of container within plus or minus 25 ml/cc of evaluator's reading

1. Explain Procedure2. Ensure client has shoes on before walking to scale3. Set scale to zero before weighing client4. Stand next to client to assists client while they step on the scale if needed5. Determine client's weigh6. Assist client in stepping off scale 7. Record weight based on indicator on scale. Weight is within plus of minus 2 lbs. of evaluator's8. Wash Hands
